The system contains a great deal of confidential information that creates a necessity for security monitoring and audit trails at levels that will achieve acceptable confidence levels and abilities to analyze any discrepancies.
Audit Records: Various activities will cause the creation of audit records that will provide a trail to investigate activity affecting the database as well as being a base for management reports. The three major types of audit records are "Access", "Usage", and "Change". System Access: The initial access to the system is through the input of a USER ID and PASSWORD. The system will record, through an "access audit record", access attempts and indicate success/non-success. Should a USER ID/PASSWORD combination fail three times in succession, the system operator terminal will receive a detailed warning message indicating a possible security problem. Should no activity occur for a period of time (to be determined) the system will automatically sign-off the user and, in the case of dial-up access, disconnect the line. The period of inactivity may vary by each user or user group. The manner in which USER IDs and Passwords are assigned and the effective periods of validity and the lengths of dormancy allowed before automatic disconnect will be topics of the installation conferences. The advantages and disadvantages of the various methods will be discussed in detail before the Department establishes policy in these regards. System Usage: Once access is achieved, the system will record in a "usage audit record", at a minimum, date and time of access, access line information (including caller id), identifying information of those records accessed, and date, time and type of sign-off. On-line Updates to Records: For every occurrence of on-line record maintenance, updates or additions, the system will create a "change audit record" indicating the date, time, USER ID, data before and data after.
